Lottery Win Claim Forms
In most States you can claim lottery prizes of up to $600 at participating retailers. However, for prizes over $600 you may need to complete a Claim Form and send, or take, it along with your prize winning ticket along to one of the States' claim centers or Lottery District Office.

Guidlines for How to Claim Safely
- Sign the back of your ticket as soon as you purchase it, and keep it in a safe place. A lottery ticket is a bearer instrument, so if it's not signed, anyone with the ticket can claim any prize won. Fully complete any other required information on the back of the ticket.
- Fill out a claim form if required - see your state below for details. Some states also require copies of a government-issued photo ID and a document verifying your Social Security Number.
- Make a copy of all items for your records (or take a clear photo on your phone), including the front and back of the ticket. You'll need to mail or take the original ticket and claim form to the lottery.
- For claims by mail include your return address on the envelope. Use certified or registered mail to track your envelope - lotteries aren't responsible for lost claims.
